A valid referral and appropriate management plan are required.
Appointments at One&All Studio are not bulk billed, so an out-of-pocket cost applies.

What is a GP Chronic Condition Management Plan?
A GP Chronic Condition Management Plan is a structured plan developed by your GP to help manage an ongoing medical condition.
Your GP may recommend care from other allied health professionals, including physiotherapists and osteopaths, as part of managing your condition.
The plan replaced the older GP Management Plan and Team Care Arrangements system on 1 July 2025.
Many patients still refer to this as an EPC or CDM referral.
Am I eligible for a Medicare rebate?
You may be eligible if:
– You have a medical condition that has lasted, or is expected to last, at least 6 months or is terminal.
– Your GP determines that you would benefit from a structured management plan.
– Osteopathy or physiotherapy is clinically appropriate and included in your management plan.
– You have a valid referral and meet Medicare’s eligibility requirements.
- Persistent lower back or neck pain
- Osteoarthritis and ongoing joint conditions
- Long-term musculoskeletal or movement problems
- Rehabilitation needs associated with certain chronic medical conditions
How do I get the Medicare referral
Speak with your GP
Book an appointment with your GP and ask whether a GP Chronic Condition Management Plan may be appropriate for your condition. Your GP will assess your eligibility and whether osteopathy or physiotherapy would be helpful
Obtain a referral
If appropriate, your GP will prepare or review your management plan and issue a referral for osteopathy or physiotherapy. The referral can be sent directly to One&All Studio or provided to you
Book your appointment
Contact One&All Studio to arrange your consultation. Please provide your referral before your appointment so our team can review the details.
Attend your consultation and pay
Your osteopath or physiotherapist will assess and treat your condition. You pay the normal consultation fee.
Claim your Medicare rebate.
As at 1 July 2026, the Medicare rebate for an eligible individual osteopathy or physiotherapy consultation under this initiative is $63.40.
How many appointments can I claim?
- 3 physiotherapy appointments and 2 osteopathy appointments
- 5 osteopathy appointments
- 2 physiotherapy appointments, 2 podiatry appointments, and 1 dietitian
FAQ's
Do I need a GP referral before booking?
You can enquire or arrange an appointment beforehand, but a valid referral and the appropriate plan must be in place before the service to claim the Medicare rebate.
Is an EPC plan the same as a Chronic Condition Management Plan?
EPC is an older term frequently used to describe Medicare-supported allied health care. Since 1 July 2025, new GP Chronic Condition Management Plans have replaced the previous GP Management Plans and Team Care Arrangements. Eligible older arrangements can remain valid during the transition period.
Do I need a new referral every year?
Not necessarily. Under the current rules, referrals issued from 1 July 2025 are generally valid for 18 months from the first service under the referral, unless the referring practitioner specifies a different period. The underlying management plan must also remain valid. Your yearly Medicare service allocation still resets in January.
Can I use my referral at One&All if it names another clinic?
Generally, you can choose another eligible practitioner within the same profession specified in your referral. Our team can review your referral before your visit.
Can I use my Medicare referral for both osteopathy and physiotherapy?
The referral must cover the relevant type of allied health service. A referral for physiotherapy does not automatically cover osteopathy. Speak with your GP if you need a referral for another profession.
Can I claim Medicare and private health insurance together?
No. You cannot use both Medicare and private health insurance extras to claim the same appointment.
What happens after my five appointments?
You may continue treatment as clinically appropriate, but Medicare will not pay further rebates under the standard five-service allocation until you have a new annual entitlement and remain eligible.
Does Medicare cover the entire appointment cost?
No. One&All Studio charges a private consultation fee. Medicare pays a fixed rebate for eligible appointments, leaving an out-of-pocket cost.
